목적 : To study the efficacy of pneumatic displacement for treating submacular hemorrhage (SMH) related to exudative maculopathies and determine prognostic factors for visual outcome.
방법 : This study involved 8 eyes with typical exudative age-related macular degeneration (AMD) and 20 eyes with polypoidal choroidal vasculopathy (PCV). All patients underwent pneumatic displacement with perfluoropropane or sulfur hexafluoride gas for SMH. The logarithm of the minimum angle of resolution best-corrected visual acuity (BCVA) was measured at diagnosis, and 3 and 6 months after treatment. Angiographic diagnosis, and tomographic features were reviewed and their correlation to BCVA at 6 months were analyzed.
결과 : BCVA at 6months(0.71±0.59) improved from baseline (1.15±0.62) after treatment (p=.001). Duration of symptom before treatment was 15.52±15.19days. Mean macular thickness, SMH height and subretinal lesion height were 113.8±56.1µm, 264.4±245.7µm and 231.8±255.7µm respectively. BCVA at 6 months was significantly correlated with baseline BCVA (p<.001), initial subretinal lesion height (p=.005) and shortest radius of SMH centered on fovea(p=.015).Subretinal lesion was thinner in PCV (p=.002)and PCV patients showed better BCVA than patients with typical exudative AMD at 6 months (p=.023).
결론 : In our cohort, vision has been improved significantly 6 months after pneumatic displacement. Patients with better baseline visual acuity, thinner subretinal lesion height and shorter radius of submacular hemorrhage resulted better visual outcome at 6months and these factors were more frequently detected in PCV patients.
