목적 : Brimonidine tartarate is a selective a2-adrenergic receptor agonist that is indicated for use in the treatment of glaucoma and ocular hypertension. We report a small number of case of bilateral granulomatous conjunctival inflammation as a late adverse reaction to long-term brimonidine therapy. This has not been previously reported.
방법 : A retrospective review was performed of patients who had prescribed brimonidine between January 2010 and May 2013. 19 patients who developed conjunctival mass like lesions while using topical brimonidine for chronic glaucoma, were clinically evaluated. Conjunctival biopsies in 2 symptomatic patients of them were taken from the lower conjunctiva(fornix).
결과 : Of the 208 patients who had prescribed brimonidine for more than 1 months reviewed, 19(9.13%)developed granulomatous conjunctivitis. Multiple nodular elevations of the inferior fornical conjunctiva were found, which were not accompanied by bulbar conjunctival injection and allergic conjunctivitis. Shortly after discontinuing the brimonidine, most of all showed full resolution of the conjunctival mass. Histologic features of the conjunctiva of 2 eyes showed atypical lymphoproliferative lesion and ones mimicked MALT lymphoma.
결론 : Granulomatous conjunctival mass should alert the ophthalmologist to potential drug toxicity in patients on topical medications such as brimonidine.
