Anterior laminar insertion depth in the eyes of primary open angle glaucoma: Swept-source optical coherence tomography study

목적 : To investigate the anterior laminar insertion depth (ALID) and its difference between horizontal and vertical meridians in the eyes of primary open angle glaucoma (POAG) using swept-source optical coherence tomography (SS-OCT). 방법 : Optic discs of 53 POAG eyes (mean age, 61.25 ± 9.52 yrs.) and 43 normal eyes with similar age (61.49 ± 9.78 yrs., P=0.90) were scanned by SS-OCT. Scans were composed of 5 horizontal and 5 vertical lines passing through the center of the optic disc. The ALID, defined by a vertical distance between the reference plane of Bruch’s membrane opening and the anterior laminar insertion, was measured, and averages of the measured values were analyzed. 결과 : The POAG eyes showed significantly larger horizontal (430.65 ± 108.67㎛ vs 366.33 ± 102.41㎛) and vertical ALID (473.12 ± 104.34㎛ vs 381.82 ± 109.87㎛) compared to normal eyes (all P<0.001). Vertical ALID was significantly larger than horizontal ALID in both POAG and normal eyes (P<0.001 and P=0.035, respectively), but the POAG eyes demonstrated larger vertical-horizontal ALID difference (P=0.009). 결론 : Anterior laminar insertion (ALI) was located more posteriorly in the eyes of POAG compared to normal eyes with similar age, as confirmed by SS-OCT. In addition, vertical ALI displaced more posteriorly than horizontal ALI in the eyes of POAG, which may support higher susceptibility to damage of superior and inferior nerve fibers than horizontal fibers in glaucoma.
