Usefulness of institution-specific measurement variability in the Detection of RNFL and Macular Thickness change using SD-OCT in Advanced Glaucomatous Eyes with Visual Field Progression

목적 : To derive the institution-specific test-retest variability using Cirrus SD-OCT and apply this information to investigate its ability to detect progressive visual field (VF) change based on Humphrey field analyzer (HFA) in advanced glaucomatous eyes. 방법 : Intersession test-retest variability of circumpapillary retinal nerve fiber layer thickness (cRNFLT) and total macular thickness (TMT) parameters were calculated from stable glaucoma subjects (control 1 group). “Significant thickness change” was defined as thickness measurement that exceeds the 95% CI of test-retest variability. The sensitivity and specificity of Cirrus SD-OCT for identification of progressive VF change were tested on progressive and stable control 2 groups using the criteria defined from the intersession test-retest variability. 결과 : All ICCs from cRNFLT and total macular thickness TMT measurements were excellent, ranging from 92.9% to 99.5% for intersession measurements. The intersession test-retest variability defined at the 95% level for cRNFLT measurements ranged from 5.24 to 8.16, while that for TMT measurements ranged from 4.56 to 7.33. The sensitivities of Cirrus SD-OCT cRNFLT measurement (In ≥ 1 quadrant in cRNFLT) was 22.7%, while that of TMT measurement (In ≥ 1 sector in TMT) was 63.6% when tested at the 95% confidence interval (CI). This difference is statistically significant (P=0.012). 결론 : Intersession measurements of both cRNFLT and TMT parameters using cirrus SD-OCT in advanced glaucomatous eyes ([MD]<-10dB) were similar to those reported in early and moderate glaucomatous eyes ([MD]≥-10dB). TMT parameters outperformed cRNFLT measurements in terms of detecting progressive VF loss in patients with advanced glaucoma.
