Interobserver agreement on Disc Damage Likelihood Scale (DDLS) and the effect of a structured training module in the interobsrever agreement.

목적 : To investigate Interobserver agreement of Disc Damage Likelihood Scale (DDLS) described by glaucoma specialist and trainees. Interobserver agreement among trainees who completed the DDLS training module was also measured. 방법 : The study comprised of 190 eyes from 190 patients. Patients were categorized into normal, glaucoma suspect and glaucoma groups. DDLS of each patients was described by trainees of different post graduate years before and after attending a training module. A total of two hours training program was designed to provide detailed information regarding the DDLS. Followed by one hour Objective Structured Clinical Examination (OSCE), one hour of lecture was given to all trainees. The weighted kappa values for the interobserver agreement was measured. 결과 : The weighted kappa value of agreement among observers was higher in all cases indicating that there were improvement in interobserver agreement after the training program. Although there were substantial variations of weighted kappa values among each pair, the increase of the kappa value was higher in junior trainees compared to senior trainees. Training program did improve interobserver concordance and the concordance at DDLS 5 and DDLS 6 was the lowest whereas a good concordance was observed between DDLS 0 and 4. 결론 : Supervised teaching program can increase the interobserver agreement of DDLS among trainees. DDLS value of 5 and 6 had the poorest interobserver concordance and caution is required when describing DDLS of these values.
