목적 : To compare the complication profile and the functional outcome of combined intraocular lens(IOL) removal and scleral fixated posterior chamber intraocular lens(SFPCL) implantation with pars plana vitrectomy(PPV) versus without PPV for dislocated intraocular lens without severe posterior segment pathology.
방법 : From January 2008 to September 2013, 19 eyes of 19 patients that underwent combined IOL removal and SFPCL implantation with PPV(Group 1) and 20 eyes of 20 patients that underwent same procedure without PPV(Group 2) were retrospectively analyzed. Parameters included duration of dislocated intraocular lens, preoperative visual acuity, postoperative complications, visual acuity and astigmatism. History of previous operation, trauma, co-existing ocular disease and other ocular abnormality were evaluated for each patient.
결과 : The postoperative mean BCVA was 20/29±0.24(SD) in Group 1 and 20/27±0.23 in Group 2; the difference was not significant (P =.560). The postoperative mean astigmatism was -1.71±1.11 in Group 1 and -2.36±1.97 in Group 2; the difference was not significant (P =.0.517). Complications occurred in 9 eyes (47.4%) in Group 1 and 13 eyes (65%) in Group 2 (P >.05). The most frequent complications in Group 1 and 2 were captured IOL, increasing of Intraocular pressure(IOP). There were 3 complications involving posterior segments only in group 2.
결론 : Combined IOL removal and SFPCL implantation with PPV seemed to provide a lower complication rate than combined IOL removal and SFPCL implantation without PPV in dislocated IOL cases. There was no significant difference in postoperative functional outcome between the group 1 and the group 2. Long-term comparison of both techniques is required.
