A case of visual loss following injection of Poly-(L)-Lactic Acid filler into the right forehead

본문 : Purpose: We report a case of visual loss after the injection of Poly-(L)-Lactic Acid filler on the right forehead area for cosmetic purpose Case report: A 46 years old female patient visited our clinic due to sudden visual disturbance and dysesthesia on right forehead and cheek. Her best corrected visual acuity was no light perception on right eye and 20/20 on left eye. Her visual acuity on right eye was not corrected. There was afferect papillary defect on the right eye, and paralysis on oculomotor muscles was observed. Fundus exam revealed pale optic nerve and turbid retina on posterior pole. The retinal vessels were narrowed. There was papule on the 1 cm to the lateral margin of right eyebrow due to the needle injection, and no other visible abnormalities of skin were observed. She had a cosmetic Poly-(L)-Lactic Acid filler injection on right forehead just before the visual disturbance occurred. Fluorescent angiography showed occlusion of right retinal artery, and there was blood flow defect on retina and choroid. She was followed up for six months, and her visual acuity and dysesthesia on right forehead and cheek sis not improved. Conclusion: There were several cases of visual loss after filler on glabella and forehead for cosmetic purpose. However, most of the complications were after the use of hyaluronic fillers, and not by Poly-(L)-Lactic Acid filler. This is the first case report of visual loss caused by Poly-(L)-Lactic Acid filler in South Korea on normal patients. Hence, it is meaningful to consider its complications when using this type of fillers.
