Fluorescein Angiography of Aggressive Posterior Retinopathy of Prematurity Treated with Intravitreal anti-VEGF in Large Preterm Babies

본문 : Purpose: To investigate the characteristics of serial fluorescein angiography (FA) findings of aggressive posterior retinopathy of prematurity (AP-ROP) treated with intravitreal anti -vascular endothelial growth factor (VEGF) in large preterm babies. Methods: Three large preterm infants (6 eyes) transferred from a secondary hospital and diagnosed with AP-ROP were included in this study. In cases 1 and 2, fundus photographs and FA were obtained before and after intravitreal anti-VEGF injection. In case 3, FA was obtained after treatment. Results: Before treatment, FA showed significant delay of choroidal filling, loop-shaped large arteriovenous shunts and capillary nonperfusions. Leakages were observed from diffuse flat neovascularization and arteriovenous shunt vessels. After treatment, leakages, flat neovascularizations and arteriovenous shunts had disappeared. Conventional ROP reactivation occurred in case 1. At 5 months after treatment, far peripheral retina did not fully vascularize and a few terminal point leakages were shown. In case 3, terminal point leakages were seen after laser treatment. Conclusion: FA showed severe choroidal and retinal nonperfusions in a large preterm baby with AP-ROP. After anti-VEGF injection, although the far peripheral retina did not fully vascularize, retinal vessels stabilized.
