Department of Ophthalmology, Veterans Health Service Medical Center
Junsung Lee, Sangkyung Choi
목적 : To determine patterns of ocular flora and factors associated with fluoroquinolone resistance in Korean population undergoing cataract surgery 방법 : Conjunctival cultures were obtained from the patients undergoing cataract surgery at VHS medical center on the day of surgery before instillation of any ophthalmic solutions. The medical records of patients with positive CNS and S.aureus cultures were reviewed to determine factors associated with fluoroquinolone susceptibility. 결과 : Of 734 CNS and S.aureus cultures, 175(23.8%) were resistant to ciprofloxacin, levofloxacin, gatifloxacin and moxifloxacin. In multivariable logistic regression analysis, use of fluoroquinolone within 3 months(OR=4.918), recent hospitalization(OR=6.646) and ocular surgery(OR=8.457) were significant predictor of fluoroquinolone resistance, along with use of fluoroquinolone within 1 year before surgery(OR=1.943), other topical antibiotic use (OR=1.673), systemic antibiotic use(OR=2.665), intravitreal injection(OR=2.976) and use of eyedrops containing benzalkonium chloride(OR=2.323). 결론 : Recent topical fluoroquinolone use, hospitalization and ocular surgery were significantly associated with fluoroquinolone resistance in CNS and S. aureus isolates from ocular culture.
