상사근 마비 및 V형 사시에서 나타나는 하사근기능항진에 대한 정량적 평가

목적 : To evaluate the amount of inferior oblique overaction using photographs in superior oblique palsy and V-pattern strabismus 방법 : Photographs were obtained in the front and adduction positions of gaze, and the images were processed using Photoshop. The images were analyzed using Image J program for the measurement of the axis of version. The axis of elevation in adduction was quantified by using a modified limbus test. Then comparement that with degree measured clinically were done. 결과 : We enrolled 100 inferior oblique overaction patients. The patients were superior oblique palsy(N = 60) and V-pattern strabismus(N = 40). The clinical grade in this study were +1(n = 40), +2(n = 40), +3(n = 15), and +4(n = 5). The mean axis of inferior oblique overaction were 5.3° ± 2.7 in grade +1 , 12.2° ± 3.5 in grade +2, 24.3° ± 3.1 in grade +3, and 35.2° ± 5.6 in grade +4. 결론 : A modified limbus test using photographs is an objective and quantitative tool for measuring ocular movement of the inferior oblique overaction. By using these merits, we measured the axis of elevation in adduction in superior oblique palsy and V-pattern strabismus. We expect this quantitative and objective evaluation can be helpful in the clinical practice.
