결절맥락막 혈관병증환자에서 유리체강내 아바스틴 주입술 3일 후의 망막구획에 따른 조기 반응

목적 : To evaluate the changes of total macular cube volume as well as the volume in three retinal compartments at 3 days after intravitreal bevacizumab (IVB) in polypoidal choroidal vasculopathy (PCV). 방법 : Twenty eight patients (28 eyes) underwent sequenced combination treatment, IVB first and photodynamic therapy (PDT) 3 days later. Spectral-domain optical coherence tomography (SD-OCT) was performed at baseline and before PDT (3 days after IVB). Total macular cube volumes as well as retinal, subretinal fluid (SRF) and subpigment epithelial (PED) cube volumes were calculated using manual delineation of all 512 x 128 series of raster scans. The cube volumes were automatically calculated by the built-in formula of SD-OCT. 결과 : At baseline, intraretinal edema (n=10, 36%), SRF (n=25, 89%) and PED (n=8, 29%) were accompanied. There was significant reduction in the mean volumes of inner retinal (P=.006) and subretinal compartment (P=.002) as well as total macular cube volumes (P=.001) at 3 days after IVB. The mean volume of PED was not change significantly (P=.67). Intraretinal edema was reduced in all of 10 eyes, and resolved completely in 3 eyes. SRF decreased in 12 cases (48%). 결론 : Intraretinal edema showed most fast responses to IVB. Our study results provide a rationale for sequenced combination treatment which may allow for more efficient delivery of PDT to consolidated target lesion, particularly PCV with predominant intraretinal edema. Further studies are warranted to determine a clinical correlation for these results.
