목적 : To analyze factors affecting the development of tight lid syndrome(TLS) in glaucoma patients and to evaluate the effectiveness of Icare rebound tonometer (Tiolat Oy, Helsinki, Finland) in measuring intraocular pressure(IOP) in eyes with TLS
방법 : 14 eyes of 14 glaucoma patients with TLS and 70 eyes of 70 age and sex matched control patients on any topical anti-glaucoma medicine.TLS was defined lack of enough interpalpebral fissure widening to measure IOP by Goldmann applanation tonometer(GAT) or inability to turn inside out of upper lid. The difference of GAT and IRT in a patient and ophthalmologic medical history (total bottle number of topical anti-glaucoma medicine, total number of sort of topical anti-glaucoma medicine, sort of anti-glaucoma medicine, period of treatment)
결과 : There was no significant difference in the gender, age, refractive error, and corneal thickness between the groups. The TLS group had definite relationship with the use of topical prostaglandin analogs (PGAs) .The TLS group had larger difference of measured IOP between GAT and IRT, more sort of topical anti-glaucoma medicine, longer period of usage of topical anti-glaucoma medicine and total accumulative dosage of topical anti-glaucoma medicine.
결론 : Patients who have been using topical PGAs combined with multiple adjuvant topical medicine for long time were at a higher risk to develop TLS. TLS may be a cause of over-estimation of IOP by GAT. IRT was effective as a reference for an estimation of real IOP in eyes with TLS.
