목적 : We will study pupillary reaction and headache after cataract surgery, according to the quantity of carbachol and acetylcholine which injected to prevent high intraocular pressure(IOP). So we want to know about the safety margin of the quantity of drugs.
방법 : 140 eyes scheduled for routine cataract surgery were randomly assigned into 7 groups, 20 eyes each. (1) Balanced salt solution infusion (Control) (2) Carbachol 0.1 cc infusion (3) Carbachol 0.5 cc infusion (4) Carbachol 1.0 cc infusion (5) Acetylcholine chloride 0.1 cc infusion (6) Acetylcholine chloride 0.5 cc infusion (7) Acetylcholine chloride 1.0 cc infusion. We compared IOP, pupil size, recovery time of pupillary reaction and pain score of headache(Verbal Rating Scale, VRS) after surgery.
결과 : There was no significant difference in IOP between 7 groups postoperatively. In acetylcholine group, pupillary reaction returned faster than carbachol group. Carbachol affects longer than acetylcholine. The more drug injected, the slower recovery of pupillary reaction in each group. 11 patients complained headache postoperatively. 8 were in carbachol group and others were in acetylcholine group. There was no correlation between duration of headache and duration of miosis.
결론 : Both carbachol and acetylcholine were effective in preventing high IOP after cataract surgery. Only 11 patients complained headache in our study, we need much more patients group in further study to define safety margin of the quantity of drug.
