목적 : We report an adult man with bilateral simultaneous optic neuritis due to anti-NMDA receptor antibody.
방법 : A 39-year-old man presented with a 3-day history of sudden visual loss and pain on eye movements in the right eye (OD). For the past 13 years, he had been admitted four times to another university hospital for altered mentation, memory impairments, and symptoms of hyperphagia, hypersexuality and hyperorality. On this visit, his best corrected visual acuities (BCVA) were light perception OD and 20/20 in the left eye (OS). Hardy–Rand–Rittler (HRR) color test showed a total dyschromatopsia OD, and nonspecific RG defects OS.
결과 : Brain magnetic resonance imaging revealed swelling of the entire intraorbital segments of both optic nerves with a strong enhancement. Serological examination for anti-NMDA antibody was positive. Intravenous methylprednisolone was administered for 5 days, and followed by oral prednisolone and azathioprine. Three months later, BCVA were 20/25 OD and 20/20 OS. HRR color test showed a total dyschromatopsia OD and mild RG defects OS.
결론 : Anti-NMDA-receptor antibody should be sought in optic neuritis accompanied by encephalitis.
