양이측반맹을 보인 선천성 Tilted disc syndrome

목적 : To report a patients with tilted disc syndrome with bitemporal visual field defect mimicking chiasmal compression from a mass lesion. 방법 : A 45-year-old man presented with decreased visual acuity of both eyes. Best-corrected visual acuity was 20/60 in the right eye and 20/40 in the left eye. 결과 : He showed bilateral multiple granular deposits on anterior corneal layer that was presumed to be Avellino dystrophy. Dilated fundus examination showed situs inversus of the major retinal vessels and inferiorly located conus in each eye, which was characteristic for tilted disc syndrome. A bitemporal visual field defect respecting the vertical meridian line in each eye was noticed on visual field examination. Subsequent magnetic resonance imaging (MRI) failed to reveal any mass lesions affecting the optic nerves or chiasm. 결론 : Tiled disc syndrome can present with visual field loss resembling that seen in chiasmal disease. In addition, this is the first case of tiled disc syndrome associated with Avellino corneal dystrophy.
