목적 : Punctate inner choroidopathy (PIC) is a rare inflammatory multifocal chorioretinopathy, which predominantly affects young myopic women. In this report, authors found a case of young female PIC patients accompanying CNV.
방법 : A 25-year-old myopic female presented with deteriorating vision in her right eye, which started two weeks ago. Her visual acuity was 20/40 (-7.75 D) on right eye and 20/20 (-9D) on left eye. On biomicroscopic examination, there were no cells in the anterior chamber and vitreous cavity. Fundus examination revealed multiple small, yellow opaque round lesions in the posterior pole. Fluorescein angiography (FA), indocyanine green angiography (ICGA), and optical coherence tomography (OCT) were consistent with foveal choroidal neovascular membrane.
결과 : She was treated with intravitreal bevacizumab (IVB) three times per month, and her visual acuity improved to 20/20 (-7.75D) on right eye, with foveal atrophic choroidal scar. Two months after the last IVB injection, visual acuity of her right eye decreased to 20/200 (-7.75 D). FA and ICGA showed more severe choroidal inflammation, so she was treated with steroid in addition to IVB. Steroid was gradually reduced through six months, and her final visual acuity was 20/22 (-7.75D)
결론 : Since PIC can be accompanied with CNV, ophthalmologists must be careful and consider the possibilities of its coexistence when treating young female CNV patients. Also when CNV is accompanied with PIC, physicians may consider IVB treatment in addition to steroid.
