목적 : To present a case of congenital tilted disc syndrome along with the results of the Cirrus GCIPL, RNFL, and ONH parameters.
방법 : A 33-year-old woman with congenital tilted disc syndrome was referred to Kim’s eye hospital for an optic disc anomaly that was detected incidentally during an evaluation for refractive surgery. Using Cirrus HD-OCT, a peripapillary retinal nerve fiber layer (RNFL), macular ganglion cell-inner plexiform layer (GCIPL) and optic nerve head (ONH) parameters were measured.
결과 : The average RNFL thickness ascertained from the OCT images was normal. The 12 clock-hour thicknesses showed inferior RNFL loss in the right eye but not in the left. However, inferior GCIPL loss was seen in both eyes. In the temporal-superior-nasal-inferior-temporal (TSNIT) OCT graph, supero-temporal, supero-nasal, and inferiotemporal RNFL peaks were observed in both eyes. The Swedish interactive thresholding algorithm (SITA) fast 24-2 of the Humphrey Field Analyzer revealed superonasal VF defects in both eyes.
결론 : In this case, the GCIPL thickness measurements showed inferior GCIPL loss especially on the temporal side in the left eye that was not detected by RNFL thickness measurements. Correlation was observed between the results obtained and defects observed on fundus photography as well as VF defects. Therefore, using OCT images for congenital tilted disc syndrome, GCIPL thickness measurements can be a more useful tool than peripapillary RNFL thickness measurements. A regular follow up of GCIPL thickness will be helpful in detecting early changes in the nerve fiber layer.
