목적 : To report the result of scleral buckle surgery without subretinal fluid drainage in patients with rhegmatogenous retinal detachment.
방법 : A retrospective review was done on 46 eyes (46 patients) with rhegmatogenous retinal detachment which underwent non-drainage scleral buckling surgery from March 2009 to August 2012 and had a postoperative follow-up period ≥ 1 year. The surgical result, including best-corrected visual acuity (BCVA) and anatomical reattachment, and the clinical course of non-drainage scleral buckling was assessed.
결과 : Mean symptom duration was 18 days, and preoperative mean logMAR BCVA was 0.55. Mean number of retinal breaks was 1.7 and retinal detachment involving fovea was seen in 24 eyes. After surgery, 39 eyes (84.8%) achieved successful reattachment and 7 eyes (15.2%) had remained subretinal fluid. In those 7 eyes, 4 eyes underwent vitrectomy, 2 eyes underwent repositioning of scleral buckle, and 1 patient discontinued follow up after 12 months. All 6 eyes which underwent additional surgery were reattached finally. Mean logMAR BCVA at 1 year was 0.30.
결론 : In patients with rhegmatogenous retinal detachment without complications such as proliferative vitreoretinopathy, scleral buckling without subretinal drainage can achieve high anatomical success rate. It can also avoid the complications of subretinal drainage, such as hemorrhage, hypotony and iatrogenic retinal tear.
