목적 : To determine the incidence and risk factors of intraoperative floppy iris syndrome(IFIS) in patients undergoing cataract surgery.
방법 : 981 eyes from 655 patients who underwent cataract surgery were enrolled and development and grade of IFIS were recorded. Correlation analysis was performed to show the relationship between the IFIS and the risk factors such as α1-adrenergic antagonist(tamsulosin, terazosin, alfuzosin), benzodiazepine, 5-α-reductase inhibitor, age, gender, hypertension, diabetes and glaucoma.
결과 : IFIS was developed in 178 eyes(18.1%) from 981 eyes. There was a correlation between the development of the IFIS and α1-adrenergic antagonist, benzodiazepine and male gender. IFIS grade tends to be higher as the cumulative dosage of the α1-adrenergic antagonist increases. Odds ratio of the patients using tamsulosin was the highest among the other risk factors, Among the both eye cataract surgery patients who were confirmed with IFIS in one or both eyes, no significant grade differences between the two eyes were noted.
결론 : α1-adrenergic antagonist and benzodiazepine was a risk factor for the development of the IFIS, and as the cumulative dosage of the α1-adrenergic antagonist increases, the probability of the higher grade of IFIS is expected to develop. Therefore, predicting and preparing for the potential IFIS to the patients who have abovementioned risk factors are necessary before planning the cataract surgery. Additionally, the aspect of IFIS on the first eye could be utilized as a predictive value for developing IFIS profile on the fellow eye.
