개방각 녹내장 환자에서 24시간 활동혈압 매개변수에 대한 해석 ; 혈압의 변동성 (선행연구)

목적 : It has been more than 10 years since blood pressure (BP) variability has been related to target organ damage. We designed this study to figure out the impacts of 24 hour ambulatory blood pressure variability (BPV) in the different severity of glaucomatous damage. 방법 : We performed 24 hour ambulatory blood pressure monitoring (ABPM) in 34 eyes of 34 open-angle glaucoma patients. BPV was calculated as nocturnal dip (ND), morning BP surge (MBPS), the standard deviation(SD) of BP, and weighted fashion SD (wSD). These BPV parameters were compared between early damage group (n=16) and moderate to severe damage group (n=18), and they also compared between age-matched early damage group (n=10) and severe damage group (n=10). 결과 : In comparison of early damage and moderate to severe damage group, ND and MBPS were not significantly different (Fisher’s exact test), however the SD of daytime BP (p=0.001) and the wSD of fulltime BP (p=0.003) were higher in moderate to severe damage group with statistical significance (Mann-Whitney test). After age-matching, the ratios of ND and MBPS, the SD of daytime BP and the wSD of fulltime BP were higher with statistical significance. 결론 : This is a pilot study result implicating that 24 hour ambulatory BPV is related to the severity of glaucomatous damage. Hence a larger scale study is needed in the future.
