인공수정체 도수계산에서 샤임플러그 각막곡률과 자동 각막곡률의 비교

목적 : To investigate accuracy of the corneal power in eyes underwent routine cataract surgery between separate measuring methods by comparing the postoperative refractive outcomes according to keratometry (K) reading disparity between Scheimpflug system (Pentacam) and conventional automated keratometer. 방법 : One hundred eyes of 100 consecutive subjects who had been performed the Pentacam and automated keratometer and received cataract surgery were enrolled. The equivalent K readings from Pentacam were compared with the automated K readings. All patients were divided into two groups, one group has more than 0.50 diopters difference of K reading of the two devices and another does not. Postoperative 3-month refractive outcomes were compared with predicted refractive errors obtained from K readings from each device and applied using the SRK/T formula. 결과 : The mean absolute error (MAE) was 0.48 ± 0.49 D using the automated K method and 0.48 ± 0.47 D using the Pentacam equivalent K method. When there was greater than a 0.50 D difference of K readings between the two devices, the accuracy of IOL calculation was significantly declined and the percentage of patients achieving a MAE within ±1.00 D of the predicted value was also significantly decreased when calculated by Pentacam K readings (P < 0.01). 결론 : K readings from the Pentacam present significant risks to increase postoperative refractive error if used in isolation for IOL calculation especially when there is the K reading disparity between Pentacam and conventional automated keratometer greater than 0.50 D.
