유리체절제술을 받은 환자에서 인공수정체 경공막걸이술 시행시 무봉합 공막내 함입술

본문 : Purpose: To introduce a new technique of intrascleral pocket procedure of transscleral fixation (TF) of intraocular lens (IOL) in post-vitrectomized eye. Methods: Two points 180° apart marked at the limbus. A 2 mm-sized intrascleral pocket was created by lamellar dissection using a crescent blade 2 mm behind the limbus without conjunctival dissection. A 2.8 mm clear corneal incision (CCI) was made anterior limbus using a keratome. Prolene sutures were exteriorized through the CCI pocket with a hook. A 3-piece foldable acrylic IOL was injected with the injector system via CCI and the ends of the haptics were exteriorized through the CCI, and then prolene sutures for each haptics were tied. After achieving well-centration of IOL, ends of prolene were sutured at the intrascleral pocket bed. Knots were buried under scleral flaps to prevent exposure. Results: All patients had no complaints like conjunctival irritation and their uncorrected visual acuity was almost identical to preoperative best-corrected visual acuity at 1 day postoperatively. IOLs were well placed without tilting or subluxation. They had no wound dehiscence or endophthalmitis postoperatively. Conclusion: The technique of intrascleral pocket procedure of TSF without the need for conjunctival dissection is a successful method of sulcus fixation, thus helping in salvaging the conjunctiva for any future trabeculectomy in post-vitrectomized eyes predisposed to developing glaucoma.
