본문 : Purpose : To demonstrate surgical management of one step rescue and sutureless glued intrascleral haptic fixation of a dislocated IOL. Method : 2.5 mm x 2.5mm partial thickness scleral flap was created at inferior and superior limbus.The 23 gauge trans pars plana vitrectomy set up was prepared.The 20 gauge MVR blade was used to create a sclerotomy at 1.5mm from the limbus under the previously created scleral flap. Bimanual maneuvers using 23 gauge endgrasping forceps under chandelier illumination enabled one step rescue of IOL from the posterior vitreous cavity with one hand and simultaneous haptic externalization through sclerotomy with the other hand. 3 mm long intrascleral tunnel is created using bent 26 gauge needle and externalized haptic is placed into the intrascleral tunnel. Fibrin glue(Tissel) is used to fixate haptics and close the scleral flaps. Result : The patient’s preoperative visual acuity was counting finger. At postoperative day 1, the IOL was well centered and visual acuity improved to 0.3. The patient remained free of any complications associated with the surgery and maintained BCVA of 0.8 at postoperative 3 months. Conclusion : Simultaneous rescue and sutureless intrascleral haptic fixation of a dislocated IOL using bimanual maneuver under chandelier illumination is an effective, safe and minimally invasive surgical method to rescue and fixate the dislocated IOL without having to explant it.
