Treatment outcome of intra-arterial thrombolysis for central retinal artery occlusion |

목적 : To investigate treatment outcomes of intra-arterial thrombolysis (IAT) for acute non-arteritic central retinal artery occlusion (CRAO) and perform subgroup analyses according to CRAO stage.방법 : For 101 patients with acute non-arteritic CRAO including 57 and 44 patients in the IAT and conservative treatment (CT) groups, a retrospective review of medical records was performed. CRAO was categorized as incomplete and complete CRAO based on funduscopic and angiographic findings. Between the IAT and CT groups, the final best-corrected visual acuity (BCVA) and early (≤3 days) and final (1 month) reperfusion (improvement of retinal perfusion) rate on fluorescein angiography were compared. 결과 : Overall visual outcomes did not significantly differ between groups, whereas early reperfusion was greater in the IAT group (73.1% vs. 42.3%, P = 0.013). In incomplete CRAO, the IAT group showed greater visual improvement (1.09 ± 0.48 vs. 0.27 ± 0.53 logarithmic minimum angle of resolution [logMAR], P = 0.011). In complete CRAO, there were no significant differences in visual outcomes between the 2 groups.결론 : IAT is helpful for early restoration of retinal perfusion in CRAO. However, this procedure may be functionally beneficial only for incomplete CRAO in the usual clinical setting. |
