The Growth of small choroidal melanocytic lesion after intravitreal Bevacizumab injection

목적 : To describe the clinical effect of intravitreal bevacizumab injection (IVB) for small choroidal melanocytic lesions with subretinal fluid and to suggest possible adverse effect of bevacizumab in choroidal melanocytic lesions.방법 : Medical records of 34 patients were reviewed for risk factors of growth of choroidal melanocytic lesion, focusing on bevacizumab. We calculated tumor growth rate in 3 patients and observed clinical course of small choroidal melanocytic lesion after IVB for the purpose of decreasing SRF.결과 : Of 34 patients, 13 patients had symptomatic SRF with decreased visual acuity and 10 of them treated with IVB for the purpose of decreasing SRF. IVB had minimal effect in reducing SRF and six patients showed growth of tumors during 24.5 months. Lesions that demonstrated growth had significantly higher rate of history of IVB, symptom, thicker tumor, orange pigment and SRF. The tumor growth rate accelerated after serial IVB in 3 cases. Histopathologic finding of enucleated eye with enlarged tumor after IVB confirmed necrotic type choroidal melanoma, which is considered that related to IVB.결론 : Intravitreal injection of bevacizumab seems to be inefficacious in resolving subretinal fluid accompanied with small choroidal melanocytic lesion. Moreover, the use of bevacizumab in choroidal melanoma should be considered carefully because of the possible adverse effects of IVB of accelerating tumor growth.
