Comparison of the argon green laser and high-frequency radio-wave electrosurgery for conjunctivochalasis

목적 : To compare argon green laser and high-frequency radio-wave electrosurgery for the treatment of conjunctivochalasis. 방법 : Study population : Sixty-four eyes from 38 patients with mild, moderate, or severe conjunctivochalasis (CCh) who were refractory to conventional therapy were randomly assigned to 2 groups.
Intervention: One group was treated with the argon green laser, and the other group was treated with high-frequency radio-wave electrosurgery.
Main outcome measures: The ocular symptoms and signs, including the conjunctivochalasis grade, the tear Ocular Surface Disease Index (OSDI), the tear breakup time (TBUT), the Schirmer’s test results, and the abnormal ocular surface findings were evaluated. 결과 : The average grade reduction was 1.15 in the laser group and 2.1 in the electrosurgery group (p=0.085). The differences between preoperative and postoperative results on the OSDI, TBUT and Schirmer’s tests were 15.53, 1.06, and 0.03, respectively, in the argon laser group and 27.11, 1.87, and 0.19, respectively, in the electrosurgery group (p=0.932, p=0.419, and p=0.477). The incremental cost effectiveness ratio (ICER) was $50.90 in the laser group and $69.29 in the electrosurgery group. 결론 : Conjunctivoplasty using either the argon green laser or the high-frequency radio-wave is a simple and effective choice for the conjunctivochalasis. In terms of cost-effectiveness, the argon green laser is equally effective and more cost-effective in patients with mild grades of CCh, whereas, in patients with moderate grades of CCh, the argon green laser is less effective and slightly more cost-effective than the high-frequency radio-wave electrosurgery. However, in severe cases of conjunctivochalasis, high-frequency radio-wave electrosurgery is more effective and cost-effective.
