목적 : To elucidate the effects of postmenopausal estrogen replacement therapy (ERT) on cataract, glaucoma, and pterygium.
방법 : Data were analyzed from 3968 postmenopausal women aged over 50 years from the fourth Korea National Health and Nutrition Examination Survey (KNHANES IV), which was performed from 2007 to 2009. Participants were interviewed for socioeconomic and gynecologic factors; attended an ophthalmologic examination; and provided a blood sample for risk factor assessment.
결과 : Of 3968 postmenopausal women enrolled, 3390 have never received estrogen and 578 were receiving estrogen. The prevalence of cataract in the subjects enrolled was 44.2% in ERT group, and 63.3% in non-ERT group which was significantly different (p<0.001). IOP was significantly higher in ERT group which was in a normal range, and the prevalence of increased IOP over 21mmHg were not significantly different. The prevalence of RNFL defect were higher in non-ERT group which was statistically different. The incidence overall pterygium incidence was 4.5% in ERT group and 11.9% in non-ERT group, which was statistically significant (p<0.0001). After estimating the percentage in persons with pterygium, the rate of atrophic pterygium was significantly lower in non-ERT group (87.9 % vs. 50.1%) and flesh pterygium was significantly higher in non-ERT group (12.1% vs. 41.3%). The incidence of diabetic retinopathy was 1.2% and 4.7% in ERT and non-ERT respectively.
결론 : This study showed that postmenopausal women without ERT had higher level of cataract, pterygium and diabetic retinopathy risk compared with postmenopausal women with ERT.
