목적 : To evaluate the clinical results of optic capture with single-piece apodized diffractive toric multifocal intraocular lenses (The AcrySof® IQ ReSTOR®) in eyes of phacoemulsification cataract and IOL surgery with posterior capsular rupture.
방법 : A 50-year-old female presented with high myopia and coexisting corneal astigmatism in both eyes, and wanted presbyopia correction. Even though carefull hydrodissection with low bottle height, posterior capsule rupture occurred in both eyes. A multifocal toric IOL was implanted in sulcus of haptics and IOL optic capture through a CCC with anterior vitrectomy in both eyes. After capturing, the IOL axis was aligned with the steep meridian of corneal astigmatism in both eyes.
결과 : Preoperative equivalent refractive error was -11.0 in both eyes. Corrected visual acuity was 20/40 in both eyes. On postoperative slit-lamp and Pentacam examination, IOLs were centered in both eyes. Corneal astigmatism was undercorrected in right eye, but new astigmatism of 1.5D with unexpected axis developed in left eye. Two weeks postoperatively, the uncorrected distance visual acuity had increased from 20/400 to 20/25 in the right eye and from 20/400 to 20/30 in the left eye. The uncorrected near visual acuity improved to Jaeger 1.
결론 : This case shows that toric multifocal IOL implantation in sulcus with optic capture can provide reasonable results and centralization even in challenging cases. Astigmatic changes need further evaluation.
