목적 : This study was to assess the short-term changes of retinal nerve fiber layer (RNFL) thickness and central subfield thickness (CST) after panretinal photocoagulation (PRP) in diabetic retinopathy. We also evaluated the effect of adjuvant intravitreal bevacizumab (IVB) in reducing PRP-related RNFL thickening and macular edema.
방법 : In this prospective randomized study, 80 eyes presenting with severe non-proliferative diabetic retinopathy (PDR) to non-high-risk PDR without macular edema were enrolled. Patients were randomized to receive adjuvant IVB with PRP (adjuvant IVB group) or PRP alone (PRP group). RNFL thickness and CST were measured at every session of PRP. And then 1, 2, 6, 10 weeks after third session PRP.
결과 : Sixty-seven eyes were completed the study. Average and all quadrant of RNFL thickness increased significantly until 1 month after first PRP and then decreased. CST increased significantly after PRP and then no significant changes in CST measurements thereafter. There were no differences in RNFL thickness, CST, and BCVA at every follow-up periods between two groups.
결론 : RNFL thickness tends to progressively increase shortly after PRP until 1 month. And then slowly decrease. Central subfield thickness tends to gradually increase after PRP and maintains thickness in 3 months of follow-up. Generally in this study, IVB is ineffective as a pretreatment for PRP in reducing PRP-related RNFL thickening and macular edema.
