초기 녹내장의 중심주변시야 결손에 대한 황반 신경절세포층 두께과 시신경 유두 주위 신경섬유층 두께의 진단력 비교

목적 : To determine whether macular inner retinal layer (MIRL) outperform circumpapillary retinal nerve fiber layer (cpRNFL) in discriminating between eyes with and without paracentral scotoma. 방법 : This study included 63 early glaucomatous eyes with (PSI group) or without (PSF group) paracentral visual field (VF) defects. MIRL, such as macular ganglion cell complex (mGCC), macular ganglion cell layer+inner plexiform layer (mGCL+) and macular RNFL (mRNFL), and cpRNFL thickness were measured using 3D OCT-2000. MIRL and cpRNFL were divided into 50 cells and 36 sectors, respectively. Discriminating ability between groups of the number of cells/sectors with abnormal thickness and average thickness in the hemisphere corresponding to the VF defects (hemi-thickness) were compared by AROCs 결과 : The number of abnormal nearest sectors of cpRNFL and all MIRL parameters were significantly smaller in the PSI than in the PSF groups (P = <0.001‒0.047), whereas no significant differences were found for average or hemi- cpRNFL thickness. The AROCs of the number of abnormal nearest cells for mGCC and mGCL+ and average hemi-thickness for mGCC, mGCL+, and mRNFL were comparable each other, and significantly higher than those of the number of abnormal nearest sectors/cells for cpRNFL parameter (P = 0.0002‒0.0063), and mRNFL parameter (P = 0.0002‒0.0191). 결론 : Regional assessment of MIRL thickness may be potentially an effective method in predicting central involvement of VF defects in early glaucoma.
